Twenty years ago, when her grandmother died from complications of diabetes, Hilda was left heartbroken — and determined to help others avoid the same outcome. She found a nonprofit called City Health Works, through which she received training in diabetes education and then started working directly with patients in her community, which proved to be a fulfilling experience.
Over time, Hilda grew with the work. As Diverge Health acquired the assets of City Health Works in 2023, she took on new roles — ultimately stepping into a leadership position on the product team, designing systems that power Diverge’s care model. Today, she leads the development of tools and workflows that support Diverge Health Coaches and incorporates her own experience as a former coach into every aspect of her work.
Her journey reflects exactly what Diverge aims to create: a path for mission-driven individuals to grow their careers while making a lasting impact in their communities.


When he was a boy, Binoy’s parents took him from Chicago to his birth country of India every summer to do volunteer work. That sense of obligation to help those less fortunate stayed with him through a career in healthcare services investing where he focused on supporting companies serving the most vulnerable. In subsequently helping build out the diversified services arm of a non-profit health plan, he deepened his appreciation for what it takes to make a direct impact on patients who need more support than the current healthcare system can provide. Looking to directly improve the lives of these patients and empower those serving them, he connected with Triple Aim Partners and ultimately co-founded Diverge Health in 2022. Today, Binoy is proud to lead the company, which currently partners with practices across four states.

Park Medical Centers has been providing high-quality primary care to patients of all ages across communities in Metro Detroit for over 60 years. In 2024, Park Medical partnered with Diverge Health to improve engagement of and support for high-needs patients. “Using their data analytic capabilities and professional expertise, Diverge Health has helped Park Medical identify and support our high-needs patients,” Jeff Brown shared, “Diverge Health not only helps with effective outreach campaigns, but follows up with intense advocacy at and after visits to ensure the needs of these patients are met.”
