Leveraging solutions for better health outcomes

Diverge Health

Only 25% of primary care providers serve a large majority of patients on Medicaid. These physicians typically need to see 30+ patients per day just to breakeven and may not have the supports to holistically serve their patients. Diverge Health brings deep infrastructure to these practices, including highly trained community health teams, administrative support, and technology. By partnering with Diverge Health, primary care practices achieve outstanding results for their patients, improve quality of care, appropriately reduce cost, and thrive in the transition to value-based payment models.

PATIENT STORY

Building momentum toward lasting health

Mike, a 59-year-old patient managing hypertension, type 2 diabetes, and congestive heart failure, was referred to Diverge by his primary care physician, Dr. George. At the time, he wasn’t consistently taking his medications or monitoring his blood sugar, and his symptoms were starting to get worse.
Mike was paired with Diverge Health Coach Lisa, who ensured he had the tools he needed, from a glucometer and blood pressure cuff to education about his conditions and medications. With Lisa’s support, Mike quickly became an engaged participant. He began monitoring his vitals daily, improved his medication adherence, and increased his physical activity. After graduating from his diabetes care journey, Mike asked to continue participating in health coaching in order to gain the skills to support his hypertension and congestive heart failure.
"Personally experiencing the impact of coaching myself, I am certain my work at Diverge is enabling our Health Coaches to make a positive difference in the communities we serve by improving the quality of patients' lives."

TEAM MEMBER STORY

Shaped by experience, scaling her reach

Twenty years ago, when her grandmother died from complications of diabetes, Hilda was left heartbroken — and determined to help others avoid the same outcome. She found a nonprofit called City Health Works, through which she received training in diabetes education and then started working directly with patients in her community, which proved to be a fulfilling experience.

Over time, Hilda grew with the work. As Diverge Health acquired the assets of City Health Works in 2023, she took on new roles — ultimately stepping into a leadership position on the product team, designing systems that power Diverge’s care model. Today, she leads the development of tools and workflows that support Diverge Health Coaches and incorporates her own experience as a former coach into every aspect of her work.

Her journey reflects exactly what Diverge aims to create: a path for mission-driven individuals to grow their careers while making a lasting impact in their communities.

CO-FOUNDER STORY

Preventing the underserved from falling through the cracks

“We are addressing one of the largest gaps in the healthcare delivery systems, driving better outcomes for underserved communities through our unique model of empowering the primary care providers that serve these patients."
Binoy Bhansali
Co-Founder & CEO, Diverge Health

When he was a boy, Binoy’s parents took him from Chicago to his birth country of India every summer to do volunteer work. That sense of obligation to help those less fortunate stayed with him through a career in healthcare services investing where he focused on supporting companies serving the most vulnerable. In subsequently helping build out the diversified services arm of a non-profit health plan, he deepened his appreciation for what it takes to make a direct impact on patients who need more support than the current healthcare system can provide. Looking to directly improve the lives of these patients and empower those serving them, he connected with Triple Aim Partners and ultimately co-founded Diverge Health in 2022. Today, Binoy is proud to lead the company, which currently partners with practices across four states.
